SMCC Baptism Form


We’re excited that you have chosen to take this next step in demonstrating your faith. Please complete the below form which will automatically be emailed to the SMCC staff when submitted.

We will then contact you with the details about the next Baptism Orientation. The orientation, which lasts approximately 45 minutes, is offered about six times throughout the year on a Sunday afternoon. In this meeting you will learn more about SMCC’s beliefs concerning Baptism, the procedure itself and what to bring the day of your baptism.

In Baptism, along with your public confession of faith is your opportunity to relate how you came to know Christ as your Savior. SMCC records brief video testimonies of everyone getting baptized. These testimonies are shown before the actual baptisms. They are also used to help others see the power of Jesus Christ to transform lives. Schedules for the recording of testimonies will be at the conclusion of the orientation meeting.

An SMCC pastor performs the baptisms. However, you may request anyone to perform your baptism as long as the person has been baptized as an adult. Any questions regarding this or other aspects of baptism will be addressed in the orientation meeting.
